This was one of the forms in the 6th grade packet last spring. If you have not completed the bus registration, or you have moved during the summer, use this link to complete the form as soon as possible.

Supplies: Supply List Teachers will work with students during the first few weeks of school to get binders and school supplies organized and communicate with families about materials. If you need support getting your child the supplies they need please contact one of the school counselors. Our Communities in School coordinator will be available during open house to help with supplies as well.

Student Clothing: School is designed to prepare students for future employment, so similar to most jobs, it is expected that students dress appropriately during the school day. As you are doing your back-to-school shopping please keep the following in mind.

  • Clothing should cover all undergarments, buttocks, chest and stomach.
  • Clothing may not advertise alcohol, tobacco, weapons, drugs or contain inappropriate language or gang related content.
  • Appropriate shoes must be worn at all times, and athletic shoes are needed for PE.

Athletics/Activity participation: In order to participate in sports or other activities throughout the school year, students must have the following items completed.

  • An activity packet through FinalForms.  These required online forms are emergency information update, concussion and sudden cardiac arrest information, student activity code, insurance waiver and assumption of risk and release form.   A school accident insurance plan can be purchased through an affordable private company. Pamphlets for this can be picked up in the main office. These forms will take approximately 20 minutes to complete. Final Forms Registration Instructions for New Student-Athletes
