We are making some adjustments to our secondary level (6th-12th) daily health ticket (attestation). Currently students are daily attesting to their health in regards to COVID-19 screening questions (symptoms, exposure, positive case, etc.). With the increase in students attending each day there has been a thoughtful adjustment to conduct a monthly attestation from family/student in regards to their clearance to attend school. At the beginning of each month (starting 4/18) parents/students will complete a monthly attestation via online form/paper printed copy (https://forms.office.com/r/zpDmPBkZxE). The student will then be visually screened in each day by trained staff at the building entrances. Any student who is unable to answer NO to all attestation questions is attesting to staying home and contacting the health room. Any students who does not pass the visual screening will be sent to the health room for further assessment.
Prior to attending school on Monday, April 19th as well as at the beginning of each month, please complete either the monthly online health ticket (https://forms.office.com/r/zpDmPBkZxE) or complete the attached document via email [email protected] or bring in the paper copy.
